A period that arrives early one month and skips the next is easy to shrug off. Stress, travel, a new workout routine and disrupted sleep can all nudge your cycle off course, and Dubai's fast pace offers plenty of all four.
But when irregular cycles become your normal, it is worth asking why. One of the most common explanations is polycystic ovary syndrome (PCOS), a hormonal condition that affects roughly 1 in 10 women of reproductive age. Many of them go years without a diagnosis. Understanding how PCOS affects your cycle, which symptoms tend to accompany it, and what PCOS testing measures is the first step towards clear answers.
What Is PCOS?
PCOS is a hormonal and metabolic condition. The ovaries produce more androgens than usual, hormones every woman makes in small amounts, and this disrupts ovulation. Insulin resistance often adds to the imbalance.
Despite the name, cysts are not required for a diagnosis. Some women show the "polycystic" ovarian appearance on ultrasound and others do not.
How PCOS Changes Your Cycle?
Regular periods depend on ovulation. When ovulation becomes infrequent or stops, cycles shift. The 2023 international PCOS guideline treats cycles longer than 35 days, or fewer than eight a year, as irregular in adults. You may also notice:
- unpredictable timing from month to month
- months without a period
- unusually heavy or light bleeding
Signs and Symptoms of PCOS
Androgens and insulin act on the skin, hair and metabolism as well as the ovaries, so symptoms often appear outside the cycle:
- excess hair on the face, chest or abdomen (hirsutism)
- persistent acne along the jawline
- thinning hair on the scalp
- weight gain around the waist
- dark, velvety skin patches on the neck or underarms
- difficulty conceiving
You do not need every symptom to justify an evaluation.
Irregular Periods Do Not Always Mean PCOS
Pregnancy, stress, major weight changes, intense exercise, hormonal contraception and thyroid disorders can all disturb the cycle. High prolactin and premature ovarian insufficiency can too. Because these conditions need different treatment, ruling them out matters as much as identifying PCOS.
How PCOS Is Diagnosed
No single test confirms PCOS. Doctors assess three features:
- Irregular or absent ovulation
- High androgens, seen as physical signs or in blood results
- Polycystic-appearing ovaries
In adults, irregular cycles combined with high androgens can be enough for a diagnosis without an ultrasound, once other causes are excluded. Where ovarian appearance needs assessing, an AMH blood test can be used instead of ultrasound in adults. AMH should never be read alone as a diagnosis.
What PCOS Testing in Dubai Involves
Assessment starts with your cycle history and symptoms, followed by blood tests. Hormone testing in Dubai for suspected PCOS commonly covers:
- Androgens: total and free testosterone, SHBG, and sometimes DHEA-S or androstenedione
- LH and FSH: the hormones that drive ovulation
- Prolactin and thyroid tests: to exclude look-alike conditions
- Progesterone: to check whether ovulation has occurred, and timing matters here
- Fasting insulin and glucose: to assess metabolic health, though no single insulin measure diagnoses PCOS
Some hormones must be tested on a specific day of your cycle, and some tests need fasting, so confirm both with your doctor before you book.
When Should You Consider PCOS Testing?
Speak with a healthcare professional if your periods have become persistently irregular, particularly alongside any of the symptoms above. Seek advice sooner if:
- you have missed several periods without an obvious explanation, or have had none for three months or more
- you have not conceived after 12 months of trying, or 6 months if you are over 35
- PCOS or type 2 diabetes runs in your family
Identifying the underlying cause is more useful than assuming a diagnosis from symptoms alone. PCOS is also linked to a higher risk of type 2 diabetes, so it is worth investigating even when pregnancy is not your goal.
Can PCOS Affect Fertility?
It can. Infrequent ovulation is the main reason PCOS delays conception, but it does not make pregnancy impossible. Most women with PCOS conceive once ovulation is assessed and supported.
